This application is for a puppy from our current litter. Please answer the following questions.

What gender are you applying for?

What breeds of dogs do you have, or have you had in the past?

Do you have any other animals? (Type, breed, age)

Tell us a little about yourself, where you live, what kind of space you have access to and what kind of life you can provide for one of our precious puppies.

Large breed dogs need a minimum of 5 minutes of exercise per day for every month of life up to 1 year. Can you provide this daily?

Please list the name and location of your veterinarian.

Deposits are non-refundable, unless the buyer is unable to select a puppy from the current litter due to the placement as a breeder, service dog, or show dog. Please initial that you agree.

I agree that Tiffany Hale at Puppies from Hale has the right of first refusal if I am, at any point, unable or unwilling to care for my dog. There will be no monetary exchange of money. Please initial that you agree.

I agree that I am fully responsible for providing all necessary vet care and appropriate training. Please initial you agree. 

If for any reason you cannot care for your puppy, you are eligible to bring the puppy back within the first 7 days after pickup for a refund of the purchase price of the puppy, minus the deposit. Please initial that you agree.

We are not currently shipping puppies. If you will be traveling on an airplane with your puppy we require that you fly with your puppy in the cabin of the airplane. This is for the safety of the puppy. Please initial that you agree.

Puppies are picked up at eight weeks old. If you cannot make pick up day, you may lose your picking order. Please initial that you agree.

Final payment must be made in cash at time of pickup. Please initial that you agree.

The only way to hold a spot is to place a deposit. Picking order of puppies are based upon the order of deposit received. We do not allow visitors until the puppies have had their shots and vet clearance at eight weeks of age for their safety.
